Clinical Nurse Specialist CNM2
Social Inclusion / Liaison (Homelessness)
(Under the Governance of Mercy University Hospital Cork, CLG)
The specialist CNM2 Social Inclusion / Liaison (Homelessness) provides specialist nursing advice and support as part of the wider multidisciplinary team.
Responsibilities - The post holder will be responsible for;
•S/he will work in collaboration with this team to provide a patient centred, quality, seamless service for people experiencing homelessness
•S/he will work in partnership with key stakeholders to support and empower individuals who are experiencing homelessness, to access health services and respond to specific healthcare needs on discharge or transfer to alternative places of care
•S/he will have a responsibility to audit the service and to actively participate in education and research
Eligibility Criteria - Candidates:
•Be registered in the General Division of the Register of Nurses kept by the Nursing
and Midwifery Board of Ireland {NMBI}
•Recent (Within last 3 years) Nursing managerial experience at CNM 1 level or above, in an acute hospital setting.
•Provide evidence of clinical and professional managerial ability, leadership, communication and organizational skills.
•Excellent interpersonal skills.
•Provide evidence of ongoing professional development.
•Evidence of leading clinical practice initiatives.
•Evidence of promotion of multi-disciplinary team work.
